Minor MDC inconsistency in bis-09

In bis-09, section 5.13 says in regards to what gets hashed into the
MDC: "The input to the hash function includes the prefix data
described above".

The next section, 5.14, says in regards to the body of the MDC packet:
"...NOT including prefix data...". (my emphasis)

Given the evidence of working code, I suspect section 5.14 is
incorrect.
