ESS-01 msgSigDigest Attribute

Section 2.4, steps 1 and 5 describe an authenticated attribute
called "msgSigDigest".  Looking at Russ' OID web page, I don't
yet see an OID assigned for this attribute.  That needs to be
done.

I propose the following:

id-aa-msgSigDigest  OBJECT IDENTIFIER ::= { id-aa 5 }

so that it is simply added to the end of the existing authenticated
attribute arc.
