I'm still not clear on how 4.2.3.2 matches 4.2, though. Is it the
intention that the "outermost SignedData layer" going into this process
be the "outer" signedData layer from 4.2? That is, should the search for
the outer layer already be done when we start this processing?

If it isn't, what the flowchart does to Example 5 _now_ is pass it
straight through 1->2->4 and produces S4(S3(S2(E1(S1(Original
Content)))))

What I was saying earlier was that if the outer layer is signed without
a mlExpansionHistory, then we must delve further in, but we must also
keep a copy of the original, because if we don't find an "outer" signed
layer, we have to sign the original. In a flowchart where the only
"state" is the current layer, I don't see how that's possible.
