All work fine for <To> and <Cc> but the Bcc is never procesed.
The MTA (say, sendmail) is supposed to remove any Bcc: header before
delivery. If this is a virtual domain then you need to have the MTA
pass the envelope recipient address to procmail, generally by having it
put it on procmail's command line.
