Our site recently performed a fairly major migration, and we've been
having mail issues with which I am unfamiliar since then. User foo at
bar sends mail to user baz, at our site, quux. Here's the relevant
portion of the resulting header:

[Note that I've manually prepended a single wedge to each line for
visual demarcation and to avoid possible escaping of the first From_
pseudoheader.]
Somehow, a second From_ header (line 3) is added, incorrectly listing
claiming the actual recipient as the original sender. The second From_
line is then escaped with a wedge, presumably by formail. This is
playing merry hell with the users' MUAs and procmail recipes. Subject
lines aren't displaying in elm/mutt/mail, cats are sleeping with dogs,
etc.
Procmail is called by postfix, with the following line:
mailbox_command = /usr/bin/procmail -a $DOMAIN -d $LOGNAME
Don't use -d with postfix. To misquote the postfix FAQ
mailbox_command = /path/to/procmail -Y
