Regarding your procmail file, is there something in the file that
would prevent directories from being created? In my own procmail
file, if mail subdirectory wasn't present (spam, for example), it
would auto-create. With your file, it lists in the log procmail:
Error while writing to ".certainly-spam/new"

1. Have you set $MAILDIR to a proper value?

2. Did you include the code in a /etc/procmailrc or a user's
$HOME/.procmailrc
(the first is without a dot, the second starts with a dot)

3. If in /etc/procmailrc, check `man procmailrc` and look for DROPPRIVS.

4. It delivers to a name ending in a slash. Procmail then creates a
maildir directory. You can remove the slashes from the mailbox-names,
but then you need to add locking to the delivering recipes. In that
case procmail will create a standard mailbox-file. Maybe you already
had a file ".certainly-spam" around, so procmail couldn't create the
directory with the same name.

5. Which Operating System/File System?
