'mbox only'.

MH support was added very early, and maildir support was added in 3.14.

So it's *not* documented.

You misunderstood. The _sequence_of_processing_ is what you have to look in
the source to determine.  WHAT happens is documented -- for _when_ it occurs
one must check the source.

The only part of the source that seems to deal with escaping the From is 
formail...

Well, then, obviously, it isn't happening! 
You can disregard everything in this thread,
including the initial report.    <grin>
