An "include"d record can contain an "[+-?]all"
But if I have multiple "include"s, or things after the "include", I want
all of them to be parsed; not to stop at the end of the first one...

I think the idea is that if one include says ?all you may continue with the
next include (or anything with an "unknown" response returned), but if the
result is -all or +all it should stop there.
I don't exactly know if the spec says this but I seemed to get that idea.
Maybe it should be more explicit.
