RE: entity declaration error

You've bound the default namespace to the XSLT namespace, but you have no 
declaration for the prefix "xsl". Change 
xmlns="http://www.w3.org/1999/XSL/Transform"; to 
xmlns:xsl="http://www.w3.org/1999/XSL/Transform";, or remove the prefix from the 
stylesheet element.
