Normally the url attribute in the XML will be unique, but not
always. When it is not unique, I only want to process the
first node it finds. How do I do that?
Elimination of duplicates is just a special case of the grouping
problem, so look up the standard techniques for grouping, e.g. at
www.jenitennison.com/xslt/grouping
