Are you using Xalan-J or Xalan-C++?
Xalan-C++ does not support extension elements.
Sorry, I should have specified: I'm using xalan-j and -v reports
Xalan Version Xalan Java 2.2.D11.
I'm running it in a linux environment if that makes any difference.
