createQuote.html

Jing,

The current createQuote.html file refer to js or images using ../javascript
or ../image

I think if we can use the code below, it maybe more general for deployment.

I have tested that it works. And also using the pull path for baseHERF in 
custom.js 

Should I check in the modified code?
