See AVTs <http://www.w3.org/TR/xslt.html#attribute-value-templates>. You want

  <input type="{type}" name="{name}" value="{value}"> 

or you could go for something like

  <input>
    <xsl:for-each select="type | name | value">
      <xsl:attribute name="{name()}">
        <xsl:value-of select="."/>
      </xsl:attribute>
    </xsl:for-each>
  </input>
