Hi,
We are currently doing our XML to RTF conversions with the RTF Formatting Kit
(www.schema.de).
You just have to create a flat structure with <block> and <inline> elements,
whose "style" attribute
refers to a defined Word formatting rule.
Works great.

I think a common route is to use xslt to generate HTML then
use a word processor or similar tool to read teh html and save as rtf.
or XSlT's older sister dsssl could be used. Jade has rtf
output built in.
