You don't want an element a you want the text <a href=... so that is
<xsl:text>&lt;a href="</xsl:text>
<xsl:value-of select="item[1]"/>
<xsl:text>" target="_blank">&lt;U>Open image in new window&lt;/U></xsl:text>

The whole point of CDATA sections is that they contain character data,
not elements.
