We developed FormMapper, an easy to use template designer for static layouts
www.FormMapper.com
and resell DocDesign http://www.axostech.com/products/docdesign/index.htm
which works for dynamic layouts but requires more training.
Both are commercial products and use the RenderX XEP FO engine.
