[xsl] TOC of nested list elements for XHTML

Hi list

This spring we have had several questions about generation of TOC for
XHTML, generation of explicit hierarchy etc, making use of the
xsl:for-each-group element in XSLT 2.0 and its attribute
"group-starting-with".

I have made a tutorial, "TOC for XHTML with XSLT",

The templates could probably be improved?
