count returns the number of items in the selected sequence which is 1 in
your case.
The way to get the same number in a heading an in a reference to the
heading is to use the same code in both places, and evaluate it at the
same place (the referenced node) so...

<xsl:for-each select="key('fig-key', @href)">
<!-- whatever code you use on the figure to generate the caption number,

actually in xslt2 you can avoid using for-each to explictly move to the
referenced node, but i think it's simpler to think of it that way.
