When the save stylesheet is run, the first (outer) document is created
correctly, but the second (inner) document (qa.xml) lacks the xmlns:xsi

That is an odd output as it would not be well-formed.
Does it fix things if you use
<xsl:attribute name="xsi:noNamespaceSchemaLocation"
namespace="http://www.w3.org/2001/XMLSchema-instance">../../xml_utils/hcdocs.xsd</xsl:attribute>
?
