As there is no feedback from the XSL-FO process to the XSLT process, you'll have to have in the XSL-FO a sequence of page descriptions, the first of which doesn't have <fo:page-number/> and the rest of which do. Then you indicate that the flow is formatted with the number-less page first, followed by the numbered pages:

  <page-sequence-master master-name="frame-pages">
    <repeatable-page-master-alternatives>
      <conditional-page-master-reference page-position="first"
                                        master-reference="frame-no-number"/>
      <conditional-page-master-reference master-reference="frame-numbered"/>
    </repeatable-page-master-alternatives>
  </page-sequence-master>
