the from attribute is a pattern not an xpath expression, so it has teh same restrictions as match= on xsl:template

from="diagnosis | symptoms"

such a thing would effectively go up the ancestor tree from where you are to find the first element that matches either of those names, and then restrict the counting to the subtree below this node.
