xslt 2 that would be the specified behaviour, but xslt 1 gives a system
licence to ignore all prefixes on input and to use any prefixes on
output on the grounds that a pure xml namespace parser might just report
the namespaces in scope and not report the prefixes used however not
reporting prefixes would mean that using namespaces in context (such as
XSD types and xpath expressions would be unusable)
So it's pretty unhelpful behaviour but I don't think that technically
it's a bug if the tree you get back when you parse that output only
differs in namespace prefixes used.
