Hi Dave,
With regards to the issue, I am sort of happy to tell you that having
considered your recommendatations about the snippet <xsl:template
match="*@|comment()|processing-instruction|text()" mode="copy
element">..</xsl:template>, amend
a bit futhre, I have succeed in making the Transformation work in the
morning our time, when it comes to produce a PSMI file in XML format,
hereby triggering the PDF processor AHF to produce a FO, then the
resulting PDF.
However, We cannot receive the PDF that is rendered correclty, though
the PSMI appear to regain all FO objects. The layout and formatting
traits are basially lost in the PDF, except the cover pages.

My colleague in Sweden is currently looking into this for me. I guess this has
highlighted our gap in mastering the XSLT 2.0 and staying current with
the DITA OT evolution.

There are a couple of threads over the Internet that are related to
the error ,  for example, Michael commented on a similar problem,
however, I am bitter at my wit ends to work out a solution that works
in this context.


If you create an element
<xsl:element name='x'>
then add an attribute
<xsl:attribute name='y'>...</

That does what you want.

If you try (by whatever method)
to add an attribute having done something else
after creating the element, then XSLT says no.


Look for that something, and make sure the ordering is right.
