Ok, that's a rather old version. I'd strongly recommend an upgrade to at
least 6.2.0.

Well, either your ISP is being rather poor and not putting in any envelope
information, or it's being over-written by your own qmail system as that all
looks ok.
Can you try again with something more current, ideally grabbing the headers
before fetchmail touches the email.
