ISO-2022-JP draft

I'd like to see the document either explicitly state that by "ASCII" we
mean ANSI X3.4-1986 OR state that it means ISO-whatever OR that it
means any 7-bit character set.

I'd really prefer to add a reference to ANSI X3.4 to the end and require
that it shift back to US ASCII, but perhaps there are reasons that isn't
a good idea (if such reasons exist, I'd appreciate being enlightened on
what they are).

Otherwise, looks fine to me...
