2. In section 1 Overview, the Distinguished Encoding Rules 
   reference (DER) is given as X.509-88.  Was X.209-88 meant instead?

My understanding is that if you refer to "old school" DER (that is, a
pre-X.690 reference), you need to refer to the X.509 specification (DER
was not defined in X.209, only BER).
