Some of the behavior you are asking for is dependent on the HTTP server
since it is the one that is responsible for sending the appropriate
MIME type to the client.

On the MHonArc side, MS Word data is saved with a .doc extension if the
proper content-type is specified in the message.  You can use the ICONS
resource along with the "useicon" option to the external filter (see
the MIMEFILTERS resource) to have an icon show with each attachment.
The mhonarc docs have an example ICONS resource that you can copy which
should work well with Apache HTTP servers.
