Note, only the last m2h_external::filter setting will take effect.
After some checking, it appears you have discovered a bug. The
"usenameext" option is not handled properly, so the option basically
has no real effect. The bug was probably introduced awhile back
when attachment file writing code was changed to be reusable across
filters.
