Essentially, you change the GECOS field in your password file to
"Richard Coleman <foobar>", and add the variable
mmailid: 1

I'm intrigued as to why that was done that way. It wouldn't work on
systems where users can't change their gecos field, of course, but I
would hope those are rare.
Good question. That was part of the MH system, when I started working
on nmh. I didn't even know if was there for quite some time, until I
discovered it while examining the code.
I assume part of the reason for this design, is that you generally
don't want your users masquerading their username unless the sysadmin
knows about it.
I believe many of the design decisions were motivated by the
development of the MTA "mmdf". I don't know much about this MTA, but
apparently the developers at UCI were involved in its development.
