mhl shows message numbers. mhshow doesn't. show invokes mhshow if

so to fix this implies adding message number output to mhshow.
So I know I'm a little late to this party ... but I've realized that mhl
can output the message numbers, and mhl _is_ used to output the message
headers even when mhshow is being used. Perhaps the right solution here
is to adjust mhl configurations. I realize commits have already been
done here, so maybe I'm locking the barn after the horse has gotten out.
good point -- certainly should have been considered. in defense of
the commits, a) i kind of like the parallelism with the part
separators (-markform), and b) i think it would have been harder to
disable the message separator header with a show or mhshow commandline
option if it were generated by mhl. (possible, but harder)
