strace(8) shows inc is using fcntl(2)'s F_SETLK, and,
mhparam can also tell you how nmh was configured:
$ mhparam spoollocking
fcntl
That might be interesting to look at, because flock(2) might use fcntl
underneath the bonnet/hood.
I don't have any other explanation for the problems you've seen.
