I think that it's added by Sendmail, and not Procmail.
In the modern Internet environment (i.e., areas without old UUCP
software), a correctly configured sendmail will not pass or create a
"From " line, except when sendmail itself is delivering to a file via
the *file* mailer. This would normally only occur if you specified a
file in you .forward, or in /etc/aliases, e.g.:
archive-foo: /var/spool/archive/foo
If sendmail is invoking procmail, then it's using either the prog
mailer, or the local mailer, neither of which should let through a
"From " line.
