OK, to append:

  :0fwh
  * other conditions
  * ^\/Subject:.*
  | formail -I "$MATCH end-text"

To prepend,

  :0fwh
  * other conditions
  * ^Subject:\/.*
  | formail -I "Subject: start-text$MATCH"
