Answering my own question... It looks like my ISP had an incoming mail
overflow that kept procmail from writing the lock file. My wife downloaded
her email and deleted it from the server, and SpamBouncer started working! I
suspect this is the problem (though I really don't know how all this works)
because I tried using the pico editor to create a file /var/mail/harold.lock
. It complained that there was not sufficient room. So, after messing with
it (including removing the stuff from the server), it worked!

I installed spambouncer (http://www.spambouncer.org), and it worked
great for a little while. Then, suddenly, it stopped filtering the mail.
The log shows:
procmail: Lock failure on "/var/mail/harold.lock"
on all the unfiltered mail.
What did I do wrong? How can I fix it?
