If you're talking about milters, you're talking about sendmail ... and
it's certainly possible to have sendmail run messages through procmail
in the course of relaying.  In fact, this is exactly how procmail
expects to use the /etc/procmailrcs/ directory.

There are also milter-functions that are called before the SMTP DATA-
phase, so when only the envelope-data and the sending host's IP-number
are known.

AFAIK, procmail is called at the earliest just before accepting or
rejecting the DATA, so when the bandwidth is already spilled.
At that time you can still SMTP-reject the message (with a
'554 Transaction failed').
