Ack! My mistake. My statement applies if d-o-e is set to "no". I think
the confusion (for me) is that d-o-e defaults to "yes" for <xsl:text/> and
"no" for <xsl:value-of/>
disable-output-encoding never defaults to yes. You always have to set it
explictly if you want it (and it is rarely a good idea to want it).
