once you start an element node like <foo> then you can only add
attribute nodes using xsl:attribute before you have added any child
element or text nodes, once you have added childrren to foo you can not
go back and add attributes.

Your template <xsl:template name="questhead"> is generating attribute
nodes so you are presumably calling it at a point where you have not
just opened an element. this error is recoverable so some systems will
stop with an error and some will simply ignore your temnplate.

This Code was working all right in Apache, recently we started to use
RenderX.  

Neither Apache nor RenderX are XSLT systems so that doesn't really help,
the important thing is what XSLT system is being used. I would guess
that most Apache projects use xalan by default for example.
