So that means that unpredfixed elements are XSLT instructions so


        <div>
                     <!-- if this div is here, the error occurs *>
        </div>
        
will be reported as the same thing as

        <xsl:div>
                     <!-- if this div is here, the error occurs *>
        </xsl:div>


ie you have an element with local name div and namespace 
http://www.w3.org/1999/XSL/Transform
at that position.


I would guess that this was a typo and you meant not to have
xmlns at all or have it be the xhtml namesace
http://www.w3.org/1999/xhtml
