You are way off topic for this list, but 

But AFAIK, it is not possible to write a DTD that specifies the order
and requirements of the name attributes, 

this is true, DTD can not express this constraint.

perhaps a Schema was more appropriate.

W3c schema can not do this either. Relax NG schema can, or, to bring us
back on topicfor this list,  schematron schema (which is an Xpath based
schema language) could specify this.
