The (FAQ) problem is here:
<xsl:template match="@*|node()">

<xsl:apply-templates select="@*|node()"/>
</xsl:copy>
</xsl:template>
Your source does not have a namespace, so when you copy it XSL copies as
is. You can get around it by forcing a namespace, like:
<xsl:template match="@*|node()">
<xsl:element name="{local-name()}"
namespace="http://www.w3.org/1999/xhtml">
<xsl:apply-templates select="@*|node()"/>
</xsl:element>
</xsl:template>
