Can you summarize your use in more detail? There are interactions of initial-page-number and the kinds of breaks.

If you were in the middle of an odd page and you used break-before="odd-page" or break-after="odd-page" you get a blank page.

If you were in the middle of an even page and you used break-before="even-page" or break-after="even-page" you get a blank page.

I think all other combinations of using break end up with no blank page.
