Apart when the "same prefix" is "no prefix" (so redefining the
default namespace), I think a maybe more common case is the same URI
bound to different namespaces.
For instance when a document is edited by different authors, or using
different documents, or when some serializer is involved (for instance
somewhere in a Java WS stack: SOAP headers serialized by one component
and body by another).
