I satisfied this requirement for legislation publishing for a client quite simply by shading the background. They mark amended content in this fashion. The text stayed black, and my formatting of the text was unchanged, but a template rule intercepts the block first by its amended attribute, establishes a wrapper with a shaded background, then uses next-match to let the formatting continued without any required knowledge of the amendment status.

Though you say change bars are in use, remember that change bars can be styled so perhaps you could have dashed change bars to represent information differently than solid change bars.
