XSLT 1.0 does not define a serialization method for XHTML.
Mike, in his humility, forgot to say that some XSLT 1.0 processors
support extension output methods for XHTML. For example Saxon 6.5.x
supports the following:
<xsl:output method="saxon:xhtml" xmlns:saxon="http://icl.com/saxon"/>
But it's year 2015, it's really time to leave XSLT 1.0 and upgrade. :-)
