This isn't really what fetchmail is for.  Fetchmail only fetches mail.

When I've set up this type of change of address autoresponder, I've
used procmail to feed any mail received at an old address into the
vacation program.  The vacation program is then used to send the
change of address message, to insure that autoresponses are only sent
once to each email address, and that autoresponses aren't sent to
mailing lists and the like.

You've probably got both procmail and vacation already installed on
your system.  A "man vacation" will get you information on the
vacation program, and a "apropos procmail" will get you information on
the several man pages that describe procmail's somewhat cryptic
configuration language.
