Well, there is more to it than just that, but it is a good start!
Are you running NAT, an application-level proxy box, or some other
configuration? (The answers to the other questions will depend on your
answers to this). Do you have a private DNS inside the firewall? Are you
using private IP addresses for your internal network?

choice of spam killing software applications ?
You need access to an SMTP server, but in some configurations, this could
easily be the ISPs SMTP server. Of course, then all the clients need to be
able to get out through the firewall to port 25 somehow...
And yes, you need either IMAP or POP3 or some other local daemon running
(perhaps SSH, so users can run the mail command themselves!), otherwise
there is nothing listening for local clients who might ask for their emails
(the linux mail command goes directly to the file system, so it'll only run
on the console).
