FWIW, what works great for me is for fetchmail to hand the mail off
to sendmail's queue as it arrives.  Then, in fetchmail's postconnect
section, I run sendmail -q to distribute the queue.  It takes me
a couple of minutes to download 50 messages over a 56K line (that
rarely gets to 56K over my neighborhood's ancient copper infrastructure.)

I prefer to leave the spam in a trash file on my ISP and just
download the good stuff.  I check the trash once or twice a day
to flush it, and make sure my filters haven't trapped falsely.
