As I've proven with a previous post, plenty of MTAs out there are sufficiently hosed that this requirement is not (and perhaps cannot be) enforced.

        We need to walk before we can run.

 Also, SMTP requires backward compatibility with non-extended versions
 of the protocol (RFC 2821, section 4.1.1.1). That's why AMTP couldn't
 work as an SMTP extension.

        Which will seriously cut down the acceptance rate.
