RFC 3207 section 4 has this statement: "A publicly-referenced SMTP server MUST NOT require use of the STARTTLS extension in order to deliver mail locally." That makes it impossible to use as a way of combating email abuse.

Also, SMTP requires backward compatibility with non-extended versions of the protocol (RFC 2821, section 4.1.1.1). That's why AMTP couldn't work as an SMTP extension.
