It seems to me that since we agree that fileinto is useful to have in the base Sieve spec (despite being unable to be executed by the server in a POP environment), that having setflag would also be useful. On the other hand, I can think of some IMAP servers where Sieve can be implemented, including fileinfo, but where setflag couldn't be done, at least without changing the server's plug-in API.
